To enrol on any of our plans please complete an registration form in practice, requiring your bank details or alternatively please give the practice a call and we can enrol you over the phone. There is a one-off joining fee of £10 which is paid at the time of registration. If you would prefer to complete the direct debit details in your own time, we can email you. If you require any further information or would like to join one of our plans then please ask at reception.
