How do I join a Health Care Plan?

To enrol on any of our plans we complete an online registration form in practice, requiring your bank details. There is a one-off joining fee of £10 which is paid at the time of registration. If you would prefer to complete the direct debit details in your own time, we can email you a link. If you require any further information or would like to join one of our plans then please ask at reception.
